More than 25 years ago, the Centre for Neuro Skills began working with people with traumatic brain injuries. As one of the oldest post-acute rehabilitation facilities in the country, CNS is now recognized by health care, case management and insurance professionals as a leader in the field.Our long-term care program is a direct result of our knowledge and experience. We know what to realistically expect -- what a patient can do for themselves and when we need to offer help.Based on each individual's abilities, CNS programs are designed to maintain or build upon the achievements gained during prior rehabilitation.
Quality of Life
People respond best in a familiar environment. That's why CNS offers community living in a suburban setting. In this more natural environment, the patient returns to a familiar rhythm of living while developing a sense of self-worth and accomplishment.Daily routines include prescribed therapeutic activities, leisure time programs, chores associated with daily life, such as personal hygiene, meal planning, and grocery shopping. Within established guidelines, people are encouraged to accept personal responsibility, to volunteer in the community or take a job, and handle their own money. We also encourage regular family contact and participation.
Quality of Care
Depending upon the level of care, per diem charges for the program vary. This is determined following a thorough assessment, review of all medical and rehabilitation records, and a meeting with the indvidual.All care levels provide:
- 24-hour supervision/assistance for personal safety Activities of daily living (ADL's) Home and community integration
- Leisure time activities
Quality of Service
- 24-hour staffing, seven days a week. Fully-furnished two or three bedroom apartment. Balanced diet monitored by a registered dietitian. Patient participation in all self-care activities and ADL's as indicated by ability. Community-based transportation to program-sponsored activities. Case supervision and periodic reporting provided by Long-Term Care manager.
- Organized outings, social and volunteer activities according to individual patient interest and abilities.
Not Included:Admission physical
- Laboratory/radiologic services Physician charges/outside medical consultation or treatment Wheelchairs and /or other adaptive equipment or devices
- Clothing and personal products
Level I
Designed to assist patients with minimal or no behavioral problems.
Level II
Provides a higher level of staffing for patients who exhibit moderate behavioral, physical and/or cognitive problems.
Level III
Increased staffing for the patients who exhibit significant behavioral, physical and/or cognitive problems.
Level IV
One-on-one staff to patient coverage for those with significant behavioral problems.
