4.	Is the program accredited by the Commission of Accreditation of Rehabilitation Facilities (CARF) and/or the Joint Commission on Accreditation of Healthcare Organizations (JCAHO)?	Yes	No
5.	Is the program responsive to the requests of the case manager?	Yes	No
6.	Is communication, after admission, frequent and understandable?	Yes	No
7.	Does the program utilize quantitative measures for assessing treatment progress in all disciplines?	Yes	No
